Camp Elevate Summer Camp Liability Waiver & Release Agreement
Camp Elevate (“NamaSTEM, LLC”) provides youth educational programs, enrichment camps, and STEM-based activities designed to promote learning, creativity, teamwork, and hands-on exploration. To ensure a safe and positive experience for all participants, we ask that every parent or legal guardian carefully read and agree to the following terms.
1. Acknowledgment of Participation
I understand that my child will be participating in a day-camp program that may include activities such as basic crafts, science experiments, collaborative challenges, movement-based games, outdoor play, use of simple tools or equipment, and other age-appropriate STEM activities. I acknowledge that these activities may involve minor physical exertion, messy hands-on materials, or supervised equipment use.
I voluntarily allow my child to participate in all scheduled activities unless I provide written instructions stating otherwise.
2. Assumption of Risk
I understand and acknowledge that, as with any youth program, participation carries potential risks, including but not limited to minor cuts, scrapes, bumps, allergic reactions, slips and falls, or other unforeseen incidents.
I acknowledge that Camp Elevate staff takes reasonable precautions—including supervision, safety procedures, and appropriate equipment—to create a safe environment, but risk cannot be eliminated entirely. I agree to assume all such risks on behalf of my child.
3. Medical Authorization
In the event of a minor injury, I authorize Camp Elevate staff to administer basic first aid (such as bandages, ice packs, or cleaning small cuts). In the event of a more serious injury or emergency, I authorize staff to contact emergency medical services (EMS) and understand that I will be responsible for any associated medical costs.
I affirm that I have disclosed any allergies, medical conditions, behavioral considerations, or other important information about my child within the camp registration form.
4. Drop-Off & Pick-Up Responsibility
I understand that Camp Elevate camps are not a drop-in program and that my child must be signed in and out each day by myself or an authorized adult. I take full responsibility for timely drop-off and pick-up and understand that extended care, if applicable, may involve additional fees.
5. Personal Belongings
I understand that Camp Elevate is not responsible for lost, stolen, or damaged personal items. I agree to send only items appropriate for a learning environment and understand that unnecessary valuables should not be brought to camp.
6. Behavior & Participation
I acknowledge that Camp Elevate strives to create a supportive, inclusive environment. If my child’s behavior causes harm, disruption, or unsafe situations, staff may contact me to discuss appropriate next steps, which may include early pick-up. Refunds are not guaranteed in these cases.
7. Photo/Video Release
Camp Elevate takes photos and videos during camp activities, for internal documentation, educational materials, or promotional purposes. No identifying information will be shared.
8. Liability Release
In consideration of my child’s participation, I hereby release, waive, and hold harmless Camp Elevate (NamaSTEM, LLC), its instructors, assistants, volunteers, and property partners from any and all liability for injuries, damages, or losses arising from participation in camp activities, except in cases of proven gross negligence.
I agree that this release is binding upon myself, my child, and our respective heirs, executors, and legal representatives.
